Physical Examinations
 
Students registering for Athletics must have a current physical examination form on file, all other forms are included in this online registration. 
 
Physical examinations taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. Download the physical form by creating an account. PHYSICAL FORM-print
